UP MOM Program Serving: Luce, Mackinac, Alger, Schoolcraft and Delta Counties
The UP MOM Program serves pregnant and postpartum women (up to 12 months) in Luce, Mackinac, Alger, Schoolcraft, and Delta counties who have Medicaid and a history of substance use disorders. Community Health Workers (CHWs) provide personalized support, connecting moms with resources to improve health, stability, and recovery.

Who Qualifies?
Women who are Pregnant and up to 12 months postpartum with Medicaid Insurance who have experienced substance use disorders.
What We Do
UP Mom utilizes Community Health Workers (CHW) to support the needs of the whole person allowing women to focus on a health pregnancy and healthy lifestyle during pregnancy and after birth for both mom and baby.
Using Social Determinants of Health (SDOH) tools participants in partnership with their CHW identify needs, and get referrals to remove barriers to a healthy lifestyle.
Research Shows
Research evaluating the impacts of CHW’s during pregnancy has demonstrated potential benefits for women such as: Reduced Emergency Room visits, Reduced NICU stays for infants, Increased breastfeeding initiation, and improved access to preventative and social services for maternal and infant health.
Customized Care Coordination
By providing coordination and service referrals we help moms learn to effectively communicate with healthcare providers and advocate for needed community services and healthcare resources.
Supportive Service Referrals
UP MOM coordinates with other home visiting services, Medicaid health plans, local service agencies, and community based organizations as well as health care providers housed in birthing hospitals, substance use treatment providers, behavioral health, and OB/GYN offices to reduce barriers in care and increase participate engagement.
Stability Supports
Our CHW’s assist in securing basic needs to help families become staqbile and able to focus on their treatment and recovery. The work to provide effect supports to needs as they arise.
